Volunteer Registration
This webpage can be viewed only by computers on The Rockefeller University campus. It is intended for use by laboratories and resource centers that need the required registration forms for volunteers they've already recruited.
No one under 16 years old is allowed to volunteer. All University policies and procedures, including employment policies, also apply. (Please click here to review University policy regarding hosting minors in laboratories.)
Note that IACUC prohibits giving ID access to CBC for volunteers; they must be accommodated by a faculty member or other approved staff member.
These forms are NOT for:
- Minors who are high school students or anticipate graduating in the current school year. All minors must apply through Summer High School Science Outreach Program.
- Elementary, middle, and high school (K-12) teachers 2. Elementary, middle, and high school (K-12) teachers All K-12 teachers must apply through the Summer Science Outreach Program for K-12 Teachers.
- College sophomores and juniors who have not yet applied to the SURF Program. Lab placement for SURF Program participants is centralized through the Dean's Office. If college sophomores or juniors have either missed the SURF Program application deadline of February 1st or applied and been denied admission, then they may use these volunteer forms.
- College students who are under 18 years old (minors). Minors who are already in college should apply through the Summer Science Outreach Program.

- Laboratories wishing to host a volunteer must submit the completed originals of the Volunteer Registration Forms to the Science Outreach Coordinator, Amaranta Viera, in the Science Outreach Office (Box 53), who will review and submit them to LS&EH. Please make a copy of the completed forms for your records.
- LS&EH will contact the volunteer's mentor or the administrator in their host lab to provide information upon approval of the registration forms. The lab administrator then schedules the volunteer for a Security ID (http://www.rockefeller.edu/security/id).
- The volunteer is required to take the Lab Safety Orientation Class, LS&EH online training modules, and any additional training requirements outlined in their approval. All required LS& EH online training modules must be completed before beginning any benchwork. To enroll a volunteer in the class, please call Elsie Calo (x8324) in LS&EH.
- The LS&EH online training modules are accessible only through computers on the The Rockefeller University campus or an off-campus Rockefeller Unviersity VPN-enabled computer, via this link: http://inside.rockefeller.edu/lab/training/mandatory. Please review the EHS Training Requirement Matrix to see which modules are required for your lab: http://formspolicies.rockefeller.edu/getfile.php?type=Policy&file=LSEH_requirements_pdf
When - and only when - the Volunteer Registration Forms are approved by LS&EH and required training is complete, the volunteer may begin working in the laboratory.
If the volunteer expects to receive a stipend, housing, or help with a visa, the laboratory or resource center is responsible for dealing with those expectations. If the volunteer is to receive an hourly wage rather than a stipend, you must contact Human Resources.
