Form AR-11 - To report a change of address to the USCIS within 10 days.

Form I-90 - To renew an expiring or expired Green Card; or replace a lost or damaged Green Card.

Form I-102 - To replace a lost, destroyed or stolen I-94 card.

Form I-539 - To apply for a change or extension of status. The most common use for this form is the change of status of the dependent of an H-1B worker. It is also used for extension of a B-2 tourist.

Form I-612 - To apply for a waiver of the section 212(e) two-year home residency requirement by individuals in J visa status.

Form I-695 - To apply for a replacement Employment Authorization Document or Temporary Residence Card.

Form I-765 - To apply for an employment authorization document. The most common use is for F-1 Optional Practical Training, J-2 work authorization, and employment authorization requests during a pending Permanent Residence (Green Card) application.
